First Time Participant Paperwork

All students who are trying out for an intercollegiate team at Babson College must complete the following checklist before they will be eligible to participate.

1. Forms must be completed through the Babson Online Student Health (OSH) Portal. Registration is required to access the OSH Portal. If you have not already created your OSH username and password, you will be prompted to do so once you go to the site. If you continue to have problems please contact Health Services at 781-239-6363.  Once you have successfully logged into the OSH Portal, you will find the forms below under 'All New Student-Athletes'. Paper forms may be submitted through the 'Upload Documents' option within the Online Student Health (OSH) Portal. See below for detailed information about the required medical forms. Questions regarding medical paperwork, contact Rick Burr, Director of Athletic Performance at rburr@babson.edu.

2. All new students are required to have a pre-participation physical exam. This must be done prior to participation in a varsity activity. A Babson Pre-Participation Physical Form must be completed within 6 months of the first physical activity per NCAA regulations. Most team-related physical activity is on or about August 15. Any medical expenses involving the physical is the responsibility of the student-athlete. In some cases, an additional physical is required to meet the NCAA regulation of six months that is not covered by most health insurances.

4. If you are currently taking medication for ADD or ADHD you must provide complete documentation of this diagnosis as required by the NCAA.  Using the link provided below, please print this form and have your physician complete and sign it.  Please fax the form and any supporting documentation to Sports Medicine at 781-239-5056 or mail to:

 5. The NCAA requires that all NCAA Division III student-athletes have knowledge of their sickle cell trait status. All student-athletes must confirm their sickle cell trait status by submitting documentation testing was done and also submit the blood work verifying this prior to participation in any intercollegiate athletic activity. Any medical expenses to complete this testing is the responsibility of the student-athlete. The NCAA requires that all student-athletes are properly educated on the importance of sickle cell trait testing.

6. Copy of Insurance Card

A copy of your insurance card (front and back) is required for all varsity student-athletes. (Please upload a copy through the 'Upload Documents' option within the Babson Online Student Health (OSH) Portal.)

7. Sports Medicine will be sending an email to all first-time athletes with the ability to complete their baseline testing at home.  The email will be from support@impacttestingonline.com with the Subject: Your ImPACT Baseline Test link. 

**All incoming first year participants will be required to complete their ImPact baseline home testing by 08/01

  • IF you have a suspected head injury, you should NOT complete the baseline test and instead reach out to the athletic trainer assigned to your team.
  • IF you sustain a head injury after you have taken the baseline IMPACT test, then you should also reach out Rick Burr, Director of Athletic Performance at rburr@babson.edu. 

All-first-year athletes will still need to complete a SWAY balance test once you arrive at Babson College. 

8. The Sports Medicine Team may still need to contact you to clarify other information based upon your Medical History or Pre-participating Physical.

FREQUENTLY ASKED QUESTIONS

  • Where can I find the form that I need to give to my doctor? The link to the pre-participation physical form can be downloaded in PDF form from the athletics paperwork main page.
  • Can I schedule a Virtual Pre-participation?  Unfortunately, the NCAA requires all Pre-participation Physicals need to be done in-person 
  •  Do you allow me to fax my paperwork? No, we do not have fax capabilities that can handle activity that demands an abundant amount of paperwork. You must either mail your information or e-mail as a PDF.
  • I don't compete until the winter or spring seasons. Can I complete it later? All student-athletes must have their athletics paperwork submitted prior to arriving on Campus.  Even though your official practices will start later you will be starting team activities prior to your practices and you need a Pre-participation Physical to do these.
  • How do I know when all of my paperwork has been received by Sports Medicine? Your head coach will be receive updates over the summer and he/she can let you know your status.
  • Who should I contact if I experience any injuries or changes in my medical needs over the summer? Please contact Rick Burr at rburr@babson.edu with any updates in your health status. The athletic trainers are 10-month employees and therefore do not work in the office over the summer months. Rick will forward the information to the appropriate team's athletic trainer.
